To get an updated copy of your COMMIT Instructor License or to request a name change for your license please fill out this form below or email billing@ctyfitness.com.

If requesting a name change please input your changes like this, for example, if only changing your Last Name, input current First Name and new Last Name.

Please email billing@ctyfitness.com or call (800) 762-8905 x5 if you have questions before finishing the form.
